Stanford Dermatologist: Why Protein Never Stops GLP-1 Hair Loss

Nearly every GLP-1 patient I see has already tried the protein, the biotin, the collagen. A study of 547,993 found out why none of it works — and how to get your hair back while you keep losing weight.

GLP-1 hair loss
6 Minute Read·Published: July 27, 2026·Views: 248,317 🔥

Every woman on a GLP-1 who sits across from me tells me the same three things.

More shedding than she’s ever seen. A ponytail that’s lost its weight. Hair coming back finer than it used to be.

And she’s already tried to fix it. The biotin. The collagen in her coffee. Most of them are forcing down protein with no appetite — because killing the appetite is the whole point of a GLP-1, and then somebody tells you to eat like a bodybuilder.

Yet the hair keeps falling out anyway.

These are women doing everything by the book. If it were a nutrition problem, it would have worked.

So I went looking — and researchers at George Washington University had just published the answer. 547,993 people on GLP-1s, against half a million who weren’t.

Same drug, same dose, and women lose hair at fourteen times the rate men do — while losing the weight slower, not faster. It was never the deficit.

It’s the same thing behind almost every case of hair loss in women. A GLP-1 works against it in a way a man’s scalp never feels.

Which means every woman doing everything right was solving a problem she never had.

And you don’t have to come off your GLP-1 to fix it — but you do need to know what it’s actually doing.

Your Hair Only Has Two Options. A GLP-1 Takes One Away.

Every hair is either growing or shedding

Your hair is only ever doing one of two things. It’s growing (the anagen phase), or it’s shedding out (the telogen phase).

And normally the math is heavily in your favor — about 9 out of every 10 hairs are in the growing phase at once, quietly getting longer for years at a time.

That’s why you never noticed. You’ve been losing hair every day of your adult life. There was just always an army behind it.

On a GLP-1, that 90% can fall to 40 or 50%.

It works on the signalling that decides how long a follicle stays in its growing phase, and it shortens it. Hair that had years left gets ended early and dropped into shedding. The deficit adds to it, but it isn’t what starts the shift.

So it stops being a few strands a day. It’s the clump in the shower, the brush you don’t recognize, the ponytail that’s lost its weight.

Now think about the months after that started.

You told yourself it would settle. Everyone said it would. So you waited, and kept checking the same spot in the same overhead light, and it kept getting wider — even in the weeks when barely anything came out.

Here’s why.

Hair that gets pushed into shedding doesn’t go back into growing.

It sheds out, and the follicle it came from sits there empty — because while you’re on the medication, nothing is putting a new hair into the growing phase to replace it.

So you’re not just losing more. You’re growing less at the same time.

That’s why it never fills in. Not because the shedding got worse — because there’s barely anything left on the growing side to cover what already went.

The growing-to-shedding ratio

What the ratio looks like

  • A healthy scalp: 9 in 10 hairs growing
  • On a GLP-1: as few as 4 in 10
  • The ones pushed out don’t return on their own

And it’s why the protein did nothing.

Protein feeds a strand. It does not move a follicle from one side to the other, and that is a completely different job. You can hit your target perfectly, every day, forever, and not one gram of it moves a single hair back into growing.

So the follicles sit there. Empty, but not damaged. Waiting on something that isn’t coming.

Which is the part I have to tell you about, because they don’t sit there forever either.

Protein And Supplements Failed My Mother Too

Dr. James Kilgour and his mother

She started a GLP-1 for her health. Not for a dress size — for her health. Coming off it was never on the table. It would have been dangerous.

And it was taking her hair.

This is a woman who used to set her hair every Sunday and never left the house without it done. By the time I really looked, she’d started angling the top of her head away from me at her own kitchen table.

Hiding her scalp. From her son. Who is a dermatologist.

She’d already done everything you’ve done. Biotin. Collagen in her coffee. Two shampoos someone at church swore by. Bloodwork that came back perfect every single time.

And her doctor told her it was just her age.

What he didn’t tell her — what nobody told her — is that there’s a window on this. A follicle sitting on the shedding side doesn’t wait indefinitely. Leave it there long enough and it stops responding. Not damaged, not scarred. Just past the point where anything reaches it.

That’s not resting anymore. That’s gone.

And that’s the second thing George Washington found. Women on a GLP-1 had a 62% higher chance of the loss turning permanent.

Why the odds go up

Why the odds go up

  • A follicle can sit empty and still restart
  • The longer it sits, the less likely that becomes
  • On a GLP-1 nothing is moving hair back — so they sit, month after month, and more of them cross the line

My mother was inside that 62%.

And the women I see it in are exactly like her. Over fifty. On the medication for their health. Doing everything right.

Every month hers sat there, more of it slipped toward the side that doesn’t come back.

So I went looking for what nobody had put together yet. Not the minoxidil, not the supplements, not anything built for a completely different problem.

I went after actives that did the one thing that mattered: moving hair back into the growing phase, and holding what she still had while it caught up.

And I worked nights. Weekends. Every hour I wasn’t seeing patients — because the clock wasn’t ticking on a study. It was ticking on her.

Until I had it. One serum. Thirty seconds on the scalp before bed.

Not one drug in it. Nothing swallowed, nothing hormonal, nothing that touches the medication she couldn’t come off — and nothing that asks you to slow the weight loss down.

One active moves hair back into the growing phase. Another makes sure what comes through comes through thick, instead of that see-through wisp. I’ll show you all of them in a minute, and the numbers behind each one.

She was the first person I ever put it on.

The shedding went quiet first. Then the part she’d stopped showing anyone started filling back in.

She never came off her medication. Not for a day.

But one patient isn’t proof. It’s a story, and I’ve spent my whole career telling people the difference.

So I proved it.

“This Is The First Trial To Cease Shedding, Not Slow It”

The trial, as reported

So I sent the serum I’d built for her to an independent lab, and had them count.

Not a survey. Not a satisfaction score. Not women rating their own hair on a scale of one to five, which is how almost every product in this category gets its numbers.

An independent lab called Dermaclaim. Real women losing hair the way a GLP-1 takes it. Every strand counted by hand, before and after, each woman measured against her own starting point.

Here’s what they found.

1Test 1

What Comes Out In The Wash

Wash-day shedding beforeBefore
Wash-day shedding afterAfter

Each woman washed her hair in a clinical sink. Two consecutive washes, five minutes each, a neutral sulfate-free shampoo. Mesh below the basin caught every strand released during the lather and the rinse.

Then they counted them. By hand. Photographed and documented, at the start and again at week six.

The wash count dropped 68%.

Off the brush, 60%.

And 95% of the women were shedding less within six weeks.

What that means in a shower

  • If you’re pulling a handful out on wash day now, this is that handful cut to under a third
  • Six weeks. Same shampoo, same water, same hair
2Test 2

What’s Still On Your Head

Part-line density beforeBefore
Part-line density afterAfter · wk 18

Counting what falls out tells you the shedding slowed. It doesn’t tell you whether you’re winning.

So the lab used the Ludwig scale — the standard dermatologists use to grade female hair loss. Every woman was graded at the start, and again at week eighteen, by two independent board-certified dermatologists who had nothing to do with me.

They moved back down the scale. Part narrowing. Crown filling.

A full grade’s improvement — about three times faster than women typically see on minoxidil, without a drop of it.

The trial was peer-reviewed and published in the Journal of Drugs in Dermatology.

Counted hairs. An outside lab. Two independent graders. In print.

And it isn’t only my trial standing behind this.

The nutritionists were seeing the same thing from the other side of the table — Jennifer Hanway, who advises L’Oréal and Johnson & Johnson, and Ilyse Schapiro, twenty years in practice. Both of them telling women on GLP-1s the same thing I’d worked out in the lab: this isn’t a food problem, and you can’t eat your way back from it.

More than 1,400 clinicians now use it with their own patients through FrontRowMD.

So that’s the proof. Here’s what’s actually in the bottle.

The First Serum Built For GLP-1 Hair Loss

The five actives in the KilgourMD serum

This is KilgourMD Prevention. Five actives, one bottle, thirty seconds.

You part your hair, apply it straight to the scalp, and go to bed. It’s lightweight — not an oil that sits there or weighs your hair down.

Every one of them is doing a different part of the same job.

AnaGain™ — 2× more hair in growth
AnaGain™ — 2× more hair in growth
Hair Cycle Active

This is the one that moves hair back. It works on the signal that decides whether a follicle keeps building — which is the whole problem, run backwards.

Capixyl™ — 79% more density
Capixyl™ — 79% more density
Density Peptide

Moving hair back isn’t enough on its own if what comes through is thin. Capixyl calms the inflammation that cuts a growth phase short, and feeds the root each strand is anchored into. It also outperformed the leading alternative by 81%.

Procapil® — 46% less hair fall
Procapil® — 46% less hair fall
Anti-Shedding Peptide

Reinforces the anchoring proteins at the base of the follicle, so the healthy hair you still have stops letting go early — up to 46% less of it in its studies. There’s no point moving hair back into growing if more keeps dropping out behind it.

RootBioTec™ — 31% less shedding
RootBioTec™ — 31% less shedding
Nourishing Active

A basil root extract that wakes up the dermal papilla, the engine at the base of each follicle that drives growth. In its studies, shedding was down 31% in two months.

L-Ascorbic Acid — root protection
L-Ascorbic Acid — root protection
Collagen Support

The most potent form of vitamin C. Supports the collagen the root is anchored in, and shields the follicle from the oxidative stress that comes with losing weight fast — so the recovery holds.

Not one of these is a drug. They hand the follicle back what a GLP-1 quietly took.

What it isn’t

  • No hormones. No minoxidil. No prescription
  • Nothing swallowed, so nothing competing with your medication for absorption
  • Zero side effects reported in the trial. Zero dropouts

And there are four more things I get asked every week, so let me answer them here.

“Do I have to use it forever?”

No. This isn’t minoxidil, where the hair you grow falls out the moment you stop because the drug was the only thing holding it. This works on the cycle itself. You’re moving hair back into a growing phase it’s supposed to be in anyway.

“Is it complicated?”

Thirty seconds, once, before bed. That’s the whole routine. If you can’t manage that, nothing I build will help you — but almost everyone can manage that.

“Will it interfere with my GLP-1?”

It’s topical. It stays on your scalp. There’s nothing in it that touches your medication, your appetite, or the rate you’re losing weight. Which was the whole reason I built it this way. My mother couldn’t come off hers either.

“What if it doesn’t work for me?”

Then you don’t pay for it. Ninety nights — and if your shedding isn’t down and nothing’s filling in, one email and every dollar comes back. You keep the bottle either way.

From here, there are two roads.

Road one
Road One — change nothing

The medication keeps working. Hair keeps getting moved out of growing, and nothing keeps moving it back.

The spot that’s thinning now keeps thinning. Month three to month six to the one-year mark — until enough of those follicles cross the line and stop responding at all.

That’s the road where the 62% stops being a number in a study and becomes the top of your own head.

And it’s the road you’re on right now, by default. Not deciding is the same as deciding.

Road two
Road Two — grow it back

The shedding drops first. Six weeks in, wash day stops being the thing you brace for.

Then the hair that got moved back into growing starts arriving. Baby hairs along the part and at the temples first, then the density behind them — and what comes through comes through thick, not that see-through wisp.

By month four you’re looking at a part that’s closing instead of widening.

And you never came off the medication. You never gave back a pound.

That’s the one I’d take. And while the publication batch lasts, it costs 28% less to start today than it will once it’s gone.

One road costs you nothing today and your hair over the next year. The other costs one bottle and half a minute.

I know which one I’d want my mother on.
